You'll need to give the Original Examination Report to the DMV. ![]() The “Medical Examiner’s Certificate” card is yours to keep and must be in your possession at all times. Once you have those, head to your Primary Medical Doctor (PMD) or an Industrial Clinic and have them complete the “MEDICAL EXAMINER’S CERTIFICATE,” otherwise known as a DMV Physical. If you're getting the EMT license in California for the first time, you'll need to pick up the Request for Live Scan Service and the Medical Examiner’s Certificate at your local DMV.
